Liability and Public Release

I do hereby covenant and agree to release and hold harmless Ghidorzi Foundation, Inc., from and against any and all liability, loss, damages, claims, or actions (including costs and attorney’s fees) for bodily injury and/or property damage, to the extent permissible by law, arising out of participation in the Ghidorzi Green & Clean event. In the event that I have also registered minor children, this release shall apply to these minor children and I agree to supervise such minor children and to maintain control over and responsibility for such minor children during the Ghidorzi Green & Clean event. I understand participation in the Ghidorzi Green & Clean event involves rigorous physical activity and risks of physical injury, and I assume these risks for myself and any minor children I have registered. I hereby consent to emergency transportation and treatment in the event of illness or injury for myself and any minor children I registered. I hereby accept responsibility for the payment of any emergency transportation or treatment. I further certify that I am, and any minor children I have registered are, in good physical condition, and I, and such minor children, have no medical or physical conditions that would restrict our participation in this event. I hereby grant to Ghidorzi Foundation, Inc. and their legal representatives and assigns, the irrevocable and unrestricted right to use and publish photographs of me, and any minor children I have registered, or in which I, or any minor children I have registered, may be included, for editorial, trade, advertising and any other purpose and in any manner and medium; to alter the same without restriction; and to copyright the same. I hereby release Ghidorzi Foundation, Inc. and their legal representatives and assigns from all claims and liability relating to said photographs.
